Insurance and Financial Information

Insurance
Our office is committed to helping you maximize your insurance benefits. Many insurance policies have a lifetime orthodontic benefit that is distinct from regular dental insurance. As a courtesy to our patients, we will bill insurance companies for services and receive payment from them directly, but you are responsible for any cost not paid by the insurance company. Please remember that orthodontic insurance is a benefit for you or your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.


In addition to accepting most traditional orthodontic insurance, our office is a Delta Premier and Delta Preferred Provider, and we participate in the United Concordia Dental PPO plan.


Financial Policy
It is our mission to provide you with affordable, high-quality treatment that fits your budget. We offer a variety of payment options to meet your needs. Charges for treatment are determined and explained at the initial visit. They are based on the severity or complexity of the problem. Please do not hesitate to discuss our fees if you have any questions. There is no charge for the initial exam or for pre-treatment recall checkups. Most patients pay for orthodontics over the time of the treatment, financed monthly without interest. We will present different financial options and payment arrangements for your convenience. Our fee covers all procedures completed in our office.


We require a downpayment, which is due when treatment starts. Subsequent payments are due the 15th of each month. These remittances are not based on the number of appointments per month or year but represent a convenient method of budgeting the cost of treatment over a period of time. Our office payment does not have a finance charge.


For your convenience, we accept Visa and MasterCard. We extend a 3% courtesy for cash payment in full at the start of treatment. If you have a health savings account, we will be happy to provide necessary receipts so you may take full advantage of the money you have reserved for medical and dental costs.


We reserve the right to suspend and/ or discontinue active treatment if your account is more than 30 days past due. Only emergency care will be rendered until the account is current. If questions or problems arise regarding your account, please call us. Many times a simple telephone call will avert misunderstandings.


Please note that additional charges will be incurred for the following:


  • Lost or broken appliances/ retainers


  • $25 charge for each check returned for insufficient funds


  • $20 charge for payments not received by the 25th of each month


  • We reserve the right to charge for excessive missed appointments and for appointments cancelled without 24 hours notice.
